AGREEMENT REGARDING BOOSTER PUMP INSTALLATION
My property is provided with
water service by the Doe Bay Water Users Association (DBWUA) in which service
area my property is located. I desire to install a booster pump in the water
system serving my property.
I understand that a public
water supply is subject to interruptions or variations of flow and pressure and
that these interruptions or variations could be harmful to a booster pump in my
system. I also understand that there are controls, methods of operation, and
maintenance procedures that may protect my booster pump from damage resulting
from interruptions or variations of the flow or pressure in the water supply.
I acknowledge, understand,
and agree that it shall be my responsibility to install, operate, and maintain
my booster pump system in such a way as to protect it against interruptions or
variations in the public water supply and in conformance with all Department of
Health requirements.
Further, I acknowledge,
understand, and agree that the DBWUA is not in any way whatsoever responsible
for any defect or deficiency in the design, installation, control, protection,
operation, maintenance, or repair of my booster pump system.
I also acknowledge,
understand, and agree that it is my responsibility to ensure that all
subsequent owners of my property are advised of this responsibility regarding
the operation and maintenance of my booster pump system and that those
subsequent owners are advised that this responsibility shall be transferred to
them automatically upon their becoming an owner of this property.
I am providing this agreement
to the DBWUA to confirm that I have been informed of the potential for damage
to the booster pump system installed at my property and to confirm that I
accept the responsibilities described above.
I will sign and record this
document which will appear on all future title reports in order to inform
future owners of this property that an agreement exists with DBWUA regarding my
booster pump installation.
